What to say upon seeing the new moon and what to say on seeing the moon

Allāhumma bārik lanā fī Rajabin wa Sha‘bān, wa ballighnā Ramaḍān.
Context
We relate in Ḥilyat al-awliyā’ with a weak isnād from Ziyād ibn an-Numayrī from Anas 
who related that when the month of Rajab began the Messenger of Allah 
said: “Allah, bless us in Rajab and Sha‘bān, and let us reach Ramaḍān.” [6].We also relate it from the book of Ibn as-Sunnī with additions.
